Aftercare Support

Aftercare Support

The young people referred to our service often move into independent living at a much earlier age than their peers and usually without the parental or familial support and guidance other young people have access to.

Leaving home can be one of the most difficult life transitions we have to make and after long periods of living and sharing with others, young people often have mixed emotions when faced with the prospect of living alone. For this reason, we ensure all young people meet with their support worker to develop a move-on and resettlement plan and work with them to enable and encourage them to embrace and settle into their new home.
